Care Team Collaboration:
With consent, Respond Therapeutic Services can communicate with Support Coordinators, family members, carers, allied health providers, mental health services, schools, employers, housing services and other care team members.
This can assist with shared planning, clearer communication, coordinated goals and more consistent support.
Care team collaboration may include:
- Case discussions
- Goal planning
- Support strategy development
- Risk and safety planning
- Progress updates
- Report recommendations
- Plan review preparation
- Clarifying social work therapy goals within the broader support team
Support for Families, Carers & Informal Supports:
Social work support can also include psychoeducation and consultation with family members, carers and informal supports, where this is relevant to the participant’s goals and provided with consent.
This may include support to better understand disability-related needs, strengthen communication, develop practical support strategies, reduce relationship strain and create more consistent support across daily environments.
